Adolescent Recovery Services in Portland Metro

Support for teens and families navigating substance use and mental health—together.

What We Do

At 4D Recovery, we provide evidence-based treatment, peer support, and family services for adolescents ages 14–17, helping teens stay engaged in recovery while continuing to grow at home, in school, and in their community.

TREATMENT SERVICES

Designed for Adolescents (Ages 14–17)

Our programs are built specifically for teens—not adapted from adult treatment.

  • Outpatient (OP) and Intensive Outpatient (IOP)
  • Same-day assessments available in most cases
  • Evidence-based substance use and mental health treatment
  • Individual, group, and family counseling
  • Case management and care coordination

SCHOOL PARTNERSHIPS

Supporting Recovery & Education

We partner with local recovery high schools to help teens stay engaged in both education and recovery.

  • Collaboration with Harmony Academy and Rivercrest Academy
  • Coordinated support between treatment and school
  • Helps reduce barriers to staying in school while receiving care

4D Recovery provides treatment services, while partner schools provide education. Services are coordinated but delivered separately.

PEER SUPPORT & MENTORSHIP

Teens are supported by peers and mentors who understand what they’re going through.

  • Peer mentorship and recovery coaching
  • Age-specific recovery groups
  • Support building confidence, identity, and connection

FAMILY SUPPORT

Recovery works best when families are involved.

We provide support for parents and caregivers so they can better understand what their teen is going through and how to help.

  • Family counseling and support
  • Education on substance use and mental health
  • Tools for communication, boundaries, and rebuilding trust
